Not defending anyone but this photo is taken with a long focal length and perspective is flattened. His hand could be close to her knee, or not, and it would look the same
Only if her knee is a wide as her upper thigh. Also his hand is in the shadow he's casting on her ass. That hand is on her vag squarely.

A photo is a fixed point in time. We don’t know what’s happened before or after it. If it is sun lotion she likely asked him to do it.
