[DC and cities in three states allow noncitizen to vote in local elections],(https://ballotpedia.org/Laws_permitting_noncitizens_to_vote_in_the_United_States) so clearly there's a way to get it done. I suspect it's as Nougat suggests, a separate ballot for non-citizens and/or under-18 voters.
